If you pledged for 5+ charms as an add on, you'll be able to choose your first charm within the survey and the remaining 4 charms after the additional add-on section page. after you click next you'll get a questioner before submitting your surveys on your remaining charm choices!

If you already filled out the survey on kickstarter you don't need to fill it out twice unless you wanted to change your selections!
